Acne is a widespread problem for many of us during teenage years, and can persist for some well into adulthood. Unfortunately, a tendency to pick and squeeze spots leaves many acne sufferers with scarring that can range from quite mild with the odd small scar here and there, to very severe scarring that has a negative psychological effect. Rather like a final insult, after suffering for many years with active acne, the disfiguring effects of scarring can lead to a massive loss of confidence.

What Causes Acne Scarring?

The inflammation of the pustules and cysts associated with acne damages collagen, which is responsible for the elasticity in our skin. Squeezing and picking at spots and pimples simply leads to further inflammation and eventually more scarring.

Skin discolouration and pigmentary changes can also occur which is typical after a trauma or injury and is know as post inflammatory hyperpigmentation (PIH). This is more common is Asian and black skin types, but can also leave lighter skin tones with a reddish/brown tone around the scars.

Acne scars are categorized by their appearance and the damage the lesion has caused and may be described as:

Ice Pick Scars

These are common.
Small, narrow, very deep holes in the skin that look like something sharp (like an ice pick) has literally punctured the skin.

Rolling Scars

These are common.
These are caused by bands of scar damage to the deeper tissue in the skin. They give the skin a rolling or wave-like appearance and are usually wide and quite shallow.

Boxcar Scars

These are common.
These are round or oval deeper scars and may also be angular craters – not dissimilar to chicken pox scars. They are often found on the cheeks and temples..

Hypertrophic Scars

These are less common.
These are thickened boxcar scars and similar to keloid scars. They appear raised and lumpy and are usually the result of very severe cystic acne. They may diminish over time.



In our Droitwich Clinic we are often told by patients that they have tried many home treatments and that nothing works. Because acne scars can be deep and are often prolific, it is unlikely that any home remedies or lotions will help the texture or improve the scarring. Our Prescribing Nurse, Mary White has many years of experience in dealing with acne scars and the distress that the condition causes.

Acne Scar Treatments

Mary usually recommends medical needling for improvement to acne scarring. We use the eDS system by Genuine Dermaroller. which we find gives more superior results than the manual dermaroller. Even deeper scars can often be improved.

There may also be improvement, especially to boxcar scars, with subcision and dermal fillers. This involves loosening the fibrous scar tissue in the skin and creating a small “pocket” which is then filled with a dermal filler such as Restylane or Juvederm. This has the effect of raising the scar to make it level with the rest of the skin.

A useful adjunct to treatment is also laser skin rejuvenation. Collagenesis (or the regrowth of Collagen) can be encouraged with our Candela Laser and this new collagen can tighten and improve the quality of tissue under the scarring.
